Your All-State Checklist

Complete your publicity form and submit your photo by Sunday, March 2
The information on the form, which you will access here, helps us prepare media releases and the event program. As part of the form, you will upload your professional-looking headshot, such as a school or senior photo.

Submit your IRS Form W-9 by Sunday, March 2
In keeping with IRS regulations, each All-Stater must submit a W-9 to the foundation to receive their $1,500 award. You will upload your W-9 while completing your publicity form. The foundation will issue a Form 1099-MISC to honorees.

You and your special teacher will have complimentary admission. You may purchase up to six additional seats for immediate family members at a discounted rate of $40 per person. Click here to access the registration form.
